July is National Cleft & Craniofacial Awareness & Prevention Month (NCCAPM). However, NCCAPM and its partner organizations maintain a variety of programs throughout the year offering support and education services for patients and families, and promoting public awareness. If you would like your/your child's photo in our new Meet the Face of Change Photo Gallery, please complete the following form and attach your photo in an e-mail to info@nccapm.org. Only forms and photos that can be matched to a fully-executed Photography/Videography Permission and Release will be used in the gallery.
